Melasma & Pigment Program · Bend
Cosmelan® is the gold-standard depigmentation program for melasma and stubborn hyperpigmentation — a structured two-phase protocol that regulates pigment at its source rather than simply exfoliating the surface.

Before & after
Representative clinical results from the device manufacturer. Individual results vary; your plan is tailored during your visit.

Overview
Melasma and stubborn hyperpigmentation are among the most frustrating skin concerns because they are driven by overactive pigment cells, not just surface damage. Cosmelan®, developed by mesoestetic®, is a medical depigmentation method that works by regulating melanin production — fading existing discoloration while helping keep new pigment from forming.
The program is deliberately built in two phases. Phase one: a Cosmelan® 1 mask is applied in the office and worn for roughly 8-12 hours — your provider sets the exact duration for your skin — then cleansed at home. Phase two begins a few days later, when the Cosmelan® 2 maintenance cream enters a home schedule that tapers over about six months. That second phase is where lasting results are earned, and it is supervised, not left to guesswork.
Brightening typically begins within two to four weeks, with the most visible change across months two and three and continued improvement through the program. Because melasma is chronic and trigger-driven — sun, heat and hormones can all call pigment back — the protocol is paired with strict sun protection and a long-term plan your dermatologist shapes with you.
What to expect
Expect redness and tightness right after the mask comes off, followed by dryness, flaking and sensitivity as the skin turns over — usually heaviest in the first week and settling across the second. Most patients resume normal routines within days, though skin stays more sun- and heat-sensitive throughout the program.
The results curve is gradual by design: discoloration continues to fade across the months of the home protocol. Consistency with the prescribed schedule and disciplined daily sun protection are the biggest factors in how well and how long results hold.

Why a dermatologist
Melasma punishes guesswork — sun, heat and the wrong laser or peel can deepen it. A dermatologist confirms the diagnosis, rules out look-alikes, sequences Cosmelan® safely alongside other treatments, and builds the maintenance plan that keeps pigment quiet for the long run.

It is a depigmentation method that includes visible shedding, but its main action is deeper than exfoliation: it regulates the melanin production that drives melasma and hyperpigmentation. That is why it succeeds where surface peels alone plateau.
Typically 8-12 hours, with the exact duration set for your skin type and pigment severity. It is applied in the office and removed at home following our written instructions.
Most patients notice brightening within two to four weeks, with the most visible change across months two and three. Improvement continues through the six-month protocol as the home phase does its work.
Yes — it is one of the most phototype-versatile pigment treatments available, and melasma in richer skin tones is one of its specialty indications. Selection and follow-up by a dermatologist still matter, and we provide both.
Melasma is a chronic, trigger-driven condition, so honesty matters: pigment can re-emerge with sun, heat or hormonal shifts. The maintenance protocol, strict sun protection and a dermatologist's long-term plan are what keep it quiet — and we build that plan with you from day one.
Each has a role. Light-based treatments can help sun-driven pigment but can aggravate melasma if mis-sequenced. Our dermatology team determines which treatment leads, which supports, and in what order — so the plan works with your pigment biology rather than against it.
Medical & cosmetic, together
Cosmelan® is one tool in a pigment plan. Because melasma is a medical condition driven by sun, visible light, heat and hormones, the same board-certified dermatologists who guide your Cosmelan® program also treat it medically — with prescription topicals, oral tranexamic acid for appropriate patients, and careful sequencing of any laser or light.
